How to complete the Chain of Custody form:

1. Fill out areas on the form with shaded headings. Be sure to include who the report goes to, as well as who the invoice goes to (if different).
2. If an account has not been established with BBC Laboratories, please include card number (we accept Mastercard, Visa, or American Express), expiration date, and the name on the card.
3. If the analysis you need is not on the form, please write it in on one of the blank lines. Be sure to clearly indicate all tests required for each sample. Call the laboratory with any questions about analyses.
4. Please make sure that the Client Sample IDs on the Chain of Custody form match the labels on the samples.
5. Sign and date the form. Save a copy for your records and place the original form in a zip lock bag and send it with the samples.
